Data we collect if we offer you extra support
- Your name and address
- Date of birth
- Family details – Eg. who lives with you
- Email and telephone number
- National Insurance number
- Your GP and medical details
- Next of kin, Advocate and Power of Attorney details
- Photographs
- Income, expenditure and benefit details
- Your bank details (as part of the sign-up process)
- Your rent charges
- Details of any one accused of hurting you or your family for safeguarding purposes
- Details of any other support agencies you may be working with.
- Support you with your tenancy
- Enable you to live independently in your home
- Safeguard you and your family from harm
- Ensure your income is maximised, for example with claiming benefits.
- The Local Authority to assist with benefit issues and any health, wellbeing and safeguarding concerns
- The Department of Work and Pensions (DWP) to enable us support you with claiming benefits
- Providers of grants or trust funds.
- Third party providers and statutory agencies we work in association with for example: 5 Boroughs, to ensure your health needs are being met; Council for Volunteer Services (to enable us to work with voluntary agencies) and other support agencies.
- Third party service providers working on our behalf for example Secure Trading (to enable you to make payments), and other care and support providers.
- Help us develop services to better meet the current and future needs of our customers
- To target appropriate services to you offering help and support.
